A video showcasing joint health and social care for Blackburn with Darwen residents has been launched online.
The video is part of a new section on Blackburn with Darwen Borough Council’s website which explains how integrated health and social care is making life better for local residents.
Integrated care is being delivered by Blackburn with Darwen Council, NHS organisations that plan and deliver health services in the borough – Blackburn with Darwen Clinical Commissioning Group and Lancashire Care Foundation Trust – and local voluntary organisations, including Age UK.
The vision is to deliver effective, efficient, high quality and safe integrated care so that the residents of Blackburn with Darwen can live longer and live better.
