£100k investment in borough’s health and social care

Blackburn with Darwen Council has been awarded £100,000 by Health Education North West to become an Integrated Care Demonstrator Site.

Working in partnership with Blackburn with Darwen CCG, Lancashire Care Foundation Trust, Voluntary, Community and Faith Sector and East Lancashire Hospitals NHS Trust the Council will invest time and resources in staff and volunteers for projects to improve the health and social care of individuals, families and communities across Blackburn with Darwen.

Integrated health and social care teams will work across the borough directly supporting the individual needs of people to improve their care and reduce avoidable hospital admissions.

Transforming Lives locality teams including staff from Early Help, Troubled Families, Police, Youth Services and the Wellbeing Service will also work as early as possible with families and individuals to improve their health and social care ensuring residents get the right support when they need it coordinated in an efficient manner across the breadth of services.

This is an exciting opportunity to invest in development and training opportunities for staff and volunteers in the borough to enable them to be able to get appropriate and timely health and social care support to local people as early as possible. This will in turn mean people and families are able to benefit from this extra personalised approach to their health and social care needs.

 This is really positive news that will benefit a whole range of staff and volunteers working in Blackburn with Darwen, supporting them to work in a more integrated way. People of Blackburn with Darwen have told us they would like access to local teams that are familiar, communicate well and help them navigate the health and care system when they need it. Investing in our workforce should enable this to happen.
